Some numbers you can estimate. Some numbers you have to measure. My resting metabolic rate is the second kind. I know it. I know roughly what a 20 mile day costs on top of it. I eat to cover both. My last 20 miler came out to 5,300 calories and 700 grams of carbs. That is not a flex. It is bookkeeping. I have been injury free for five years. The biggest reason is that I stopped guessing at those two numbers.
The base is my measured resting rate from the cart, not an equation. The 20 miler is what that day actually cost: 5,300 calories, 700 grams of carbs. Covering 100 to 120 miles a week is why my maintenance lives between 4,500 and 7,000. A 2,000 calorie day at this volume is not discipline, it is a deficit.
I learned that from a different test. My watch said my threshold was 5:59 per mile. The lab said 5:25. Every zone built on the watch was 34 seconds off. Your watch's calorie estimate is the same kind of fiction. I blame underfueling for most stress fractures and most stalled progress. Measured RMR is the cheapest early warning you can buy.
The Cart Does Not Measure Calories
RMR is measured by indirect calorimetry. The cart does not measure calories. It measures the oxygen you consume and the carbon dioxide you produce. Prep is where most tests get ruined. Twelve hour fast, water only. No hard training for 24 hours, because a long run the day before inflates the reading. No caffeine, alcohol or nicotine for 12 hours. Ten minutes sitting still on arrival. First thing in the morning. Same time on every retest.
A body composition scan first is worth it. Then they report RMR per kilogram of fat free mass, the form every clinical threshold uses. A mask or hood goes on and the analyzer records 10 to 30 minutes, hunting for steady state. One question is worth asking before you pay. Do you calibrate gas and flow before every test? Many gym operators do not. An uncalibrated cart can be off 10 percent, which kills the point.
VO2 max is a separate protocol. A ramp, with speed or grade climbing every one to three minutes. You want time to exhaustion between 8 and 12 minutes. Past 15 your legs quit before your heart does. The number under reads. A true max means hitting two of these. A VO2 plateau. RER at or above 1.10. Heart rate within 10 of age predicted max. RPE 17 or higher. Treadmill, not bike.
The test distance runners actually want is metabolic efficiency, also called fat max. Submaximal, 45 to 60 minutes, stages of at least three minutes. They convert your gas exchange into grams of fat and carb burned per minute. A fingerstick lactate each stage makes it the best test money can buy for a runner. Stage length is the first thing to ask. A "VO2 max plus fat burn" that runs 12 minutes is not a metabolic test. It is a ramp with a decorative chart.
Measured Beats Guessed, Every Time
RMR converts to calories per kilogram of fat free mass. Then the daily target gets built additively. Never off a guessed activity factor. RMR, plus exercise, plus daily movement, plus 10 percent for digestion. This is where the test pays for itself. It hands you measured calories per hour at each pace. Not a watch estimate that runs 15 to 30 percent off.
Then comes energy availability. Intake minus exercise energy, divided by fat free mass. The 2023 IOC consensus statement on REDs calls 45 kcal per kg of lean mass adequate, 30 to 45 reduced, 30 or below low. A 60 kilogram runner at 12 percent body fat needs about 2,375 calories before replacing one training calorie. It also flags an RMR ratio under 0.90, measured divided by predicted, as a sign of suppression.
That is the trap. Calories set from a suppressed RMR lock the deficit in. A low number is a reason to eat more, not less. Heikura found elite distance athletes with low energy availability markers had 4.5 times the career bone stress injury rate. A 2025 retrospective found stress fractures in 70 percent of elite athletes with REDs versus 25 percent without.
Men are not exempt.
Now the fuel data. During exercise, an RER under 0.85 is fat dominant. FatMax, where fat oxidation peaks, is typically 0.3 to 0.6 grams per minute. It lands near 63 percent of VO2 max, at your first lactate turn point. That is your real zone 2 ceiling. Measured, not guessed off 220 minus age. If FatMax comes back 7:45 at 142 beats and your easy runs are 7:15, you are training carb dominant.
The race math is the payoff. Oxidizing 1.1 grams of fat and 3.5 grams of carb per minute at goal pace means 210 grams of carb an hour. Stored glycogen gives you roughly 500 grams. Your gut absorbs 60 to 90. That tells you when you bonk and what to carry. It is why I built up to 140 to 170 grams an hour out of a maple flask. A low RER is not the goal though. It does not predict performance, and it can just mean underfueling.
VO2 max deserves less than its reputation suggests. Recreational runners sit 40 to 50 for men and 35 to 45 for women, club runners 55 to 65, elite 70 to 85 and 62 to 75. Marathoners win at 70 to 78 while bigger engines lose to them. Economy and threshold are what separate trained runners. Low relative to your race times means you are efficient. Then 3 to 5 minute intervals are the lever. High means threshold volume is the constraint.
Almost All of This You Can Do for Free
You can predict RMR for nothing. Mifflin St Jeor for men is 10 times kilograms, plus 6.25 times centimeters, minus 5 times age, plus 5. For women, the same minus 161. Then comes the accuracy. A 2023 meta analysis of 1,430 athletes scored equations by how often they landed within 10 percent of measured. Ten Haaf 80.2 percent. Cunningham 54.1. Mifflin St Jeor 52.2. Mifflin underestimates athletes. In ultra runners the best option still misses by 276 calories a day in men, 388 in women.
The bigger issue is that no equation detects suppression. The ratio is measured divided by predicted. If all you have is predicted, it reads 1.00 and REDs is invisible. That is the one thing you cannot DIY. Everything else you can. Fourteen days of weighed food alongside morning weight will tell you plenty. If weight is stable, your intake is your maintenance. VO2 max has three free routes. Cooper's 12 minute test. The heart rate ratio method at 15.3 times max over resting. An all out 5K run through VDOT. Your watch is off 3 to 5 mL/kg/min with a chest strap, 5 to 8 on wrist optical. The trend is the useful part. The number is not. A handheld lactate meter runs around $300. It buys unlimited threshold retests. It is the best DIY purchase a runner can make.
Booking One Is Easier Than Most Runners Think
Prices vary more by metro than by brand. RMR runs $85 to $250. VO2 max $119 to $250. Metabolic efficiency with lactate, the one you want, $165 to $400. Bundling the two lands at $295 to $425. The lever nobody uses is HSA and FSA. Both tests are frequently eligible, effectively 20 to 35 percent off, and Quest's checkout takes those cards. Routing through cardiology to save money does not work. A hospital CPET runs $400 to $1,200 without the fuel data.
University labs are the best value and usually the best rigor. They are open to the public. They run long stage protocols. They hand over the raw file at the end. UC Davis Health lists RMR at $85 and metabolic efficiency plus lactate at $230. Bowerman at Oregon does VO2 max at $140 and lactate threshold at $165. Yours turns up by searching "human performance lab" with your state and VO2 max.
